Bodmin Way has two Health Coaches, Abbie Cavalera and Beth Rawlinson. Their role, based in NE Cornwall, Bosvenna and Three Harbours, is to work together with people on health matters, for example, diabetes 2 and hypertension.

 

Appointed coaches received a lot of training for the role which is a pilot scheme, initially for one year.

 

The CHWW pilot scheme based in St. Austell, means that Bodmin benefitted from the experience gained there. Now Bodmin Way is involved with the Health Coach pilot and will provide feedback for ways of improving the service in the hope it is established and rolled out to widely across the UK.

Health Coaches

 

We offer free one-on-one tailored support and activities for those managing Type 2 diabetes.

The Health Coaches at Bodmin Way can assist individuals in managing their health and lifestyle to improve their condition.

 

What is Health Coaching?

Health coaches can offer FREE one to one, tailored support and activities to help you make positive lifestyle changes, in partnership with the NHS.

A Health Coach supports you to:

  • Understand what matters most for your health

  • Set your own health goals

  • Build confidence in your choices

  • Learn practical ways to improve your wellbeing

 

How Can Health Coaching Help You?

Health Coaching can support many areas of your health and wellbeing, including:

  • Managing weight or improving nutrition

  • Increasing physical activity

  • Supporting mental health and emotional wellbeing

  • Building confidence and self-belief

  • Reducing isolation and improving social connections

  • Living well with a long-term health condition

 

By focusing on small, achievable changes, it becomes easier to build habits that last.

 

Ongoing Support – You’re Not Doing This Alone

Change is hard, especially on your own. Regular check-ins with your Health Coach will:

  • Help you stay on track

  • Offer encouragement when motivation dips

  • Adapt your plan when life or health changes

  • Help you overcome barriers with confidence

 

How Does It Work?

At the start, your Health Coach will take time to really understand you.

They will:

  • Get to know your current health, lifestyle and challenges

  • Help you set clear, achievable goals

  • Create a step-by-step plan tailored to you

  • Support you to access local groups, activities or services

  • Offer regular sessions, feedback and encouragement

  • Adjust your plan as you progress or new challenges arise

 

Exploring Different Options

There are many ways to improve health and wellbeing — and no one-size-fits-all solution.

Your Health Coach can help you explore options such as:

  • Community activities

  • Social or support groups

  • Food and nutrition support

  • Physical activity that feels achievable

  • Wellbeing and confidence-building opportunities

 

They’ll help you find what works best for you.

About This Pilot Project

This Health Coaching programme is a pilot project designed to support people living with long-term health conditions to take ownership of their diagnosis and feel more in control of their health and wellbeing.

 

The project is delivered in partnership with:

  • The NHS, Bosvena Health and 3 Harbours GP Surgeries

  • Cornwall VSF

  • WellFed

  • Bodmin Way

 

Together, we are working to offer compassionate, community-based support that complements clinical care.

 

About Bodmin Way

Bodmin Way is dedicated to enhancing the wellbeing of residents in Bodmin and the nearby villages. As a social enterprise owned by local parish churches, we have established strong relationships and facilities within the community. Bodmin Way is a leader in the government's neighbourhood health strategy. Our team of parish nurses, community health and wellbeing workers, and health coaches work closely with Bosvena Health and Cornwall's Integrated Care Board to improve health outcomes for those who are least likely to access services.

 

About WellFed

WellFed is a programme focused on improving access to good food, food confidence and community connection. It supports people to develop healthier relationships with food while addressing wider issues such as cost, access and wellbeing.

WellFed plays an important role in supporting this project by helping participants explore sustainable, nourishing approaches to eating that support both health and the wider community.
